All patients are required to fill out an intake and consent form for every service. All forms are now electronic through Jane Software and are completely confidential and accessible 24/7 prior to your appointment.
You should receive a link to your intake form at the time of booking your initial appointment.
If you do not receive your intake form(s), or have technical difficulties please contact us and we will assist you.
Cancellations
Should you require a cancellation or re-booking, please provide us with 24 hours notice.
A full fee will be applied for missed appointments.
